Hello! I run an awards ceremony for which there are around 15-18 voted categories. I need a form built that allows entrants or those submitting on their behalf to submit up to 500 words in text for judges to consider. The ideal form would: (a) Allow submitters to choose the category they're submitting in; (b) Allow them to choose their name / their company's name either from a list or by inputting free text; (c) Allow them to submit multiple times - this is important because one representative of a company might be responsible for up to 10 submissions. (If they have to refill the form multiple times this is okay, but ideally they could add multiple submissions in one form.) Other than that we only really need to collect name and email address of the person submitting. No other data required. I can provide logo and a list of categories / finalists, but I need this finished TODAY (September 28). Thank you!
