Are you passionate about supporting young people leaving care?We’re looking for a dedicated and empathetic Staying Close Transition Support Worker to join our team in Tameside.This is a rewarding opportunity to make a real difference in the lives of young people as they transition to independent living.About the Role:You’ll work directly with young people referred to the Staying Close service, helping them build the skills, confidence, and support networks they need to thrive. From developing tailored support plans to facilitating access to education, employment, and community activities, you’ll be a key part of their journey to independence.Location: TamesideSalary: £13.01 per hourContract Type: 3 months temporaryStart Date: ASAPKey Responsibilities:Engage young people and build trusting, empowering relationships.Conduct support needs assessments and develop individualised plans.Support access to education, training, employment, and community services.Deliver group work sessions and positive activities.Liaise with external agencies and advocate for young people’s needs.Maintain accurate records and manage a caseload effectively.About You:You’ll have:NVQ Level 3 in Care of Children and Young People / Adult Social Care (or equivalent).Experience working with cared-for children, care leavers, or vulnerable young people.Strong communication, planning, and organisational skills.Knowledge of safeguarding, child protection, and relevant legislation.A full UK driving licence and access to a vehicle for business use is desirable.Why Join Us?Competitive salary and benefits.Opportunities
