Your new role: This is a temporary Tenancy Support Worker role, providing flexible cover across services when needed. You'll work alongside people experiencing housing or wellbeing challenges, offering person led, trauma-informed support that empowers them to create meaningful, sustainable change. Your responsibilities will include:Carrying out needs assessments to understand each person's circumstances, strengths, and goals.Co-producing Individual Support Plans that promote independence and reflect personal aspirationsReviewing support plans and risk assessments to ensure they remain relevant and empoweringWorking collaboratively with local partners, housing providers, and community organisationsOffering advice and guidance through community drop-ins across Blaenau GwentCoordinating long-term and crisis interventions, including attending joint meetings and case reviewsSupporting people to access information, digital tools, and resources to manage housing, benefits, and wellbeingProviding resettlement support for those moving into new or independent accommodationMaintaining accurate case notes and sharing information appropriately with colleaguesProviding flexible outreach support in people's homes and within the community What you'll need to succeed: Knowledge of trauma-informed practice, attachment theory, and relationship based approachesExperience supporting people experiencing emotional distress or work
