About the role We are seeking a passionate Service Coordinator to lead an established teams of Person-Centred Support Workers at our Regency Green Transitional Facility. In this role, you will play a pivotal role in driving meaningful impact by mentoring and inspiring frontline staff to deliver high-quality, services. You will support a team to assist individuals with complex psychosocial and disability needs by successful transition into the community by enabling opportunities, fostering independence, and encouraging meaningful community connections. Working under the guidance of the General Manager, and in collaboration with participants, and other key stakeholders, you will help co-design and implement therapeutic, person-centred supports. You will also contribute to service evaluation to ensure effectiveness and positive participant outcomes. Based in Regency Park this role involves support to frontline teams, monitor service quality, check participant wellbeing and safety, and complete site audits. Key Responsibilities Participate in the development and review of participant plans and programs Monitor and assess plans and plan implementation for effectiveness to ensure outcomes are met, including meeting goals Lead active support and negotiate service offerings within boundaries Ensure personalised development plans are in-effect for team members Ensures supports are effectively and properly documented in accordance with funding requirements Design and innovate Roster that fits within funding framework Forward planning of rosters and risk management in establishing suitable workforce management practices Monitor and report on rostering costs, and facilitate activities within the budget Approve timesheets in accordance with payroll processes to ensure accuracy of payment of wages and entitlements Approve leave, ensuring coverage Run reports to support workforce analysis Communicate and report to key stakeholders Complete internal and external incident management reporting/ investigations Required Skills and Experience Experience in developing and coordinating delivery of complex service plans and projects to support our Participants Strong understanding of therapeutic service models, and positive behaviour support. Understanding of projects and mental health/hospital discharge planning and pathways will be an advantage Demonstrated commitment to person-centred, inclusive, and rights-based support approaches. Excellent communication skills, along with strong administrative, multi-tasking, and problem-solving abilities. Experience in independently managing site operations and service delivery, aligned with organisational policies and procedures. Ability to lead by example, inspire trust and high performance within teams. Proficiency in Microsoft Office and administrative systems that support effective service delivery.
