This is an exciting opportunity for an accomplished pharmacists to deliver high-quality, patient-focused care, while also contributing to service development and clinical leadership within the department. Key Responsibilities: Provide ward-based clinical pharmacy services, ensuring the safe and effective use of medicinesUndertake clinical screening of prescriptions, verifying appropriateness, interactions, and adherence to Trust and national guidelinesOrder and coordinate medications for patients, supporting timely and efficient dischargeDeliver medicines reconciliation, review, and optimisation for a diverse range of patientsOffer specialist advice to medical and nursing teams, supporting decision-making in complex clinical casesSupervise and mentor junior pharmacists, trainees, and techniciansContribute to the development of clinical guidelines, audits, and quality improvement projects Essential Requirements: GPHC RegistrationPrevious experience in hospital pharmacy, including ward-based clinical workConfident working collaboratively within a multidisciplinary teamStrong clinical judgement, commun
