About the Role We’re seeking a passionate Registered Nurse to deliver high-quality clinical services to NDIS participants from our Regency Park Transitional Care support service. You’ll complete health assessments, develop care plans, provide specialised training, and support participant independence through goal focused skill development. The role also includes supervising and assessing staff, contributing to their development, and ensuring safe care. Key Contributions Work closely with leaders and participants to identify health concerns and develop support strategies Coordinate, monitor and review participant health support planning and daily provision, liaising with medical practitioners to ensure optimal health outcomes. Provide clinical oversight to ensure participants access medical appointments and other interventions Develop and review therapeutic care plans, applying CLO’s risk framework. Provide clinical and nursing expertise on intake meetings to ensure seamless transition of care Ensure that hospital discharge/other reports and instructions, medication supplies and therapeutic care plans are complete on entry Identify the need for, plan and deliver training in health support procedures within staff scope of practice, applying specialist and technical knowledge Ensure compliance with medication administration requirements, reviewing and responding to incidents, with individual and team training and competency checks Undertake and report on medication and health related audits. Have knowledge of and apply the Australasian Triage Scale for participants with acute mental health conditions Develop and deliver both formal and informal training for staff training. Provide clinical oversight to participants in the service. Ability to work as part of the community nursing team and the wider CLO, About You We’re seeking a compassionate and skilled Registered Nurse who is passionate about person-centred support and committed to supporting individuals with disabilities to live their best lives. You’ll be confident working both independently and as part of a multidisciplinary team, with strong clinical knowledge, excellent communication skills, and a proactive approach to education and training. You’ll bring a commitment to quality, safety, and continuous improvement, and be aligned with CLO’s values.
