• Undertake holistic assessments and develop transition plans that will inform the appropriate pathway and support for the young person
• Co work with children's services and discharging the care act duties

To be considered for the Social Worker role:

• 1+ years experience of working within a similar social work setting
• Diploma/Degree in Social Work
• Registration with Social Work England
• Evidence of working with a complex caseload
• Experience of application of the Care Act 2014
