- The Patient Flow Coordinator plays a key role in supporting the Transitional Care Unit located in Amherstview, Ontario. This position is ideal for a regulated health professional who brings clinical expertise, strong organizational skills, and a commitment to improving patient transitions across care settings. Reporting to the immediate supervisor, the Patient Flow Coordinator is responsible for ensuring the delivery of high-quality care and service while maintaining compliance with internal and external standards of practice.
- This role requires a proactive and collaborative approach to managing patient flow. The successful candidate will be expected to assess patients, coordinate admissions and discharges, and identify opportunities for process improvement. The Patient Flow Coordinator works closely with interdisciplinary teams to ensure patients receive timely, appropriate care in the most suitable setting.
- What You’ll Be Doing
- -Manages admissions, discharges, and transfers to ensure seamless transitions and optimal bed utilization
- -Builds strong relationships with hospital partners and internal care teams to support timely, patient-centered care
- -Applies critical thinking and creativity to remove barriers to flow, address risks, and escalate concerns when necessary
- -Provides support to staff and patients through coaching, rounding, and collaborative problem solving
- -Contributes to continuous improvement initiatives and helps shape a responsive and flexible care system
- What You Bring
- -Holds a degree or diploma in Nursing, Social Work, or Social Service Work or an equivalent combination of education and experience
- -Has five to ten years of clinical experience in healthcare settings
- -Demonstrates strong leadership, facilitation, and decision-making abilities
- -Maintains a calm and collaborative approach in high-pressure situations
- -Possesses a valid drivers license and current CPR certification
