Outreach Relief Support Practitioner – Role Overview

Our Outreach service delivers personalised, community-based support that empowers individuals to live independently. We assist with daily tasks such as shopping, appointments, and household management, helping people build confidence and self-reliance.

We also promote social inclusion through activities like cinema trips, café visits, and community events—encouraging meaningful connections and a fulfilling lifestyle.

Key Responsibilities:

>Healthcare Support: Assist with personal care and medication to ensure well-being.
>Promoting Independence: Follow tailored Support Plans to encourage personal growth.
>Record Keeping: Maintain accurate documentation of support and progress.
>Collaborative Working: Engage with individuals, families, and colleagues to deliver high-quality care.

Our service runs 7 days a week with flexible shifts between 9:00am and 6:00pm.
