  • The Health Coach in our service works in close collaboration with Support Coordinators and service managers, as well as all our brilliant partner providers in Rushmoor. You will use an asset based coaching approach to provide the necessary tools and information to assist people to build their own personal resilience, improve lifestyle choices and prepare for independence by building on their own individual skills. You’ll be part of Home Group in which motivation and creativity is essential as is resilience and a huge sense of humour.
  • Jay will give you the freedom to make your role your own and evolve it to best suit the customers’ needs while also being imaginative, energised, professional and outcomes focused. It’s a great opportunity to work with diverse customers.
  • What you’ll do
  • You’ll transform lives by coaching customers as part of a multi-disciplinary team alongside our support and care colleagues as well as with external partner organisations.
  • Coaching our customers who are experiencing mental health issues to take control and ownership of their health.
  • You’ll carry out assessments and create individual health plans to enable our customers to adopt healthy behaviours. Focusing on what is strong, not what’s wrong.
  • Deliver interventions to support customers to achieve their health goals identified in their plan.
  • You’ll be a change maker there to motivate others to kick their unwanted habits and activate personalised healthy lifestyles!

  • You have
  • Experience working in the community supporting people, ideally with their health
  • Experience coaching others
  • Knowledge of the inequalities and barriers that impact on health and wellbeing
  • Ability to deal with conflict and challenging behaviour
  • If you have an NVQ in Health and Social Care, Health Trainer and/or Coaching qualification then that will be the cherry on top!
  • You’ll need to travel around to spend time with your customers, you can do this role using public transport or if you drive, to get from A to B, you’ll need a vehicle insured for business purposes. The great news is that we’ll pay your mileage between site visits!

  • The practical bits
  • Working hours Monday to Friday 09.00-17.00,
  • Although you will be lone working the customers you will have daily contact and communication with the rest of the team and we do get together for team meetings, training and often, just a cuppa.
  • You’re able to use technology. We have a variety of systems we use for reporting, creating support plans, emails, Teams calls and so on
  • You’ll be given the latest IT Kit, which could include a Surface Pro Tablet and Smart Phone, making it easier to complete support plans on the go.
  • You’ll need an Enhanced DBS check done and we pay for that.
