I am seeking a skilled freelancer to develop a GDPR-compliant patient form that allows patients to input multiple medications per submission. The form should integrate seamlessly with Airtable or Google Sheets for data storage and automation. I’m a UK pharmacist setting up a small system to help my patients manage their repeat prescriptions more easily. Right now, I use Google Forms and Sheets, but it’s limited by only allowing patients to add information about one medication per form. I’d like a clean, functional replacement where a patient can: Fill in their details once, Add details about multiple medications in a single submission, Upload a repeat-slip photo or NHS App screenshot, and Have all of that automatically saved in a spreadsheet or Airtable database. This isn’t a large company project. I just want something simple, reliable, and GDPR-safe that I can use day to day.
