Overview I am a patient managing complex chronic health conditions (Long COVID and related issues). Over the past decade I have accumulated a large number of medical test results. I need all of these consolidated into one clean, structured spreadsheet so I (and my doctors) can track results, trends, and abnormal findings. Confidentiality and accuracy are essential. What I will provide: Approx 30 Raw lab reports in PDF, Word, and other formats (Google Drive links provided). What you will deliver: A single Excel or Google Sheets file. One row per test marker. All data entered in chronological order (2015–2025). Standardized formatting across labs so the same biomarker (e.g. Sodium, CRP) can be tracked consistently. Conditional formatting to highlight abnormal, borderline, high-normal, and low-normal results. Ability to filter by date, category, test name, or abnormal results. Columns required (must be included): Date of Test Test Name (as written in report) Standardized Test Name (common name) Result Value Units Lower Reference Range Upper Reference Range Out of Range (Y/N, with conditional formatting) Lab/Source Reference range rules: Results below 10% or above 90% of reference range should be flagged as “Low Normal” or “High Normal.” Pathogens in microbiome tests should be marked “Out of Range” when present. Units = % of sample where applicable. Additional requirements: Ensure consistency in units across tests (e.g. always pg/mL, mmol/L). Ensure chronological order.
