Looking for a freelancer to convert 21 PDF documents into fillable forms. Each document contains 6 large text boxes and a section with check boxes, ranging from 9 to 14. The ideal candidate will have experience with PDF form creation and attention to detail to ensure all forms are correctly converted and fillable. See an example of the form attached here. I believe it can be completed within 1 hour. I would like to set a 'test task' to complete 1 form first, before securing the job and proceeding.
