As an Adult Social Worker in the CHC Team, your key roles and responsibilities will include:

• Conducting comprehensive assessments of individuals' health and social care needs to determine eligibility for NHS Continuing Healthcare funding.

• Working collaboratively as part of a multi-disciplinary team (MDT) to review and assess complex cases, including participation in MDT meetings and best interest meetings.

• Applying National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care guidelines consistently and accurately in all assessment and decision-making processes.

• Liaising effectively with individuals, their families, carers, and a wide range of professionals (e.g., healthcare providers, care home staff, GPs) to gather information and ensure holistic care planning.

• Developing and reviewing person-centred care plans that address identified needs and promote independence and well-being.

• Advocating for individuals to ensure their voice is heard and their rights are upheld throughout the CHC assessment and review process.

• Managing a caseload of CHC assessments and reviews, prioritizing effectively to meet deadlines and service demands.

• Preparing clear, concise, and analytical reports and documentation to support funding decisions and review processes.

• Identifying safeguarding concerns and taking appropriate action in line with Newcastle Council's safeguarding policies and procedures.

• Providing advice and guidance to individuals and families regarding the CHC process and available support options.

• Maintaining accurate and up-to-date case records in compliance with professional standards and organisational requirements.

Key Requirements:

• You must be a Qualified Social Worker with a degree in Social Work or equivalent.

• Social Work England registration is essential and must be current.

• An enhanced DBS check will be required for this role.

• Previous experience in adult social care, particularly with assessment or Continuing Healthcare processes, is highly desirable.
