💼 About the Role:

As a Social Worker within the Learning Disabilities Team, you'll be working closely with adults and their families to promote independence, safeguard wellbeing, and support person-centred care planning. You'll play a vital role in assessments, reviews, and multi-agency working, ensuring that support packages are tailored and empowering.

✅ Key Responsibilities:

• Conducting Care Act assessments and reviews

• Creating and implementing person-centred care plans

• Working in partnership with health professionals, families, and support providers

• Safeguarding vulnerable adults through robust risk assessments and protection plans

• Maintaining accurate case records in line with statutory responsibilities

🧠 Ideal Candidate:

• Qualified Social Worker (Degree/DipSW/CQSW)

• Current registration with Social Work England

• Experience working with adults with learning disabilities

• Strong understanding of the Care Act, MCA, DoLS, and safeguarding procedures

• Able to work autonomously and as part of a collaborative team
